Welcome, I’m so honored to support you on your healing journey. This form outlines the terms of our work together and ensures that we are aligned, supported, and protected, spiritually and practically. Please read and complete it before your first session.

SOUL FRESH HYPNOSIS CLIENT AGREEMENT

This Agreement is entered into between Soul Fresh Hypnosis (“Practitioner”) and the undersigned client (“Client”).

By purchasing and participating in services, Client agrees to the following terms:

1. SERVICES PROVIDED

Soul Fresh Hypnosis provides hypnosis-based services designed to support behavioral, emotional, and mindset shifts.

Services are offered in the following containers:

30-Day Intensive

• Duration: 30 days from purchase

• Includes: Access to up to three (3) hypnosis sessions

• Focus: One clearly defined issue or goal

100-Day Journey

• Duration: 100 days from purchase

• Includes: Access to up to ten (10) hypnosis sessions

• Focus: Deeper, layered challenges or ongoing support

These services are results-focused containers, not session-based packages. Sessions are utilized as needed to support the Client’s outcome.

2. SESSION STRUCTURE & DELIVERY

• Sessions are scheduled by the Client within the active service period

• Session frequency, length, and format are flexible and determined at the Practitioner’s discretion based on Client needs

• “Up to” session limits represent maximum access, not guaranteed usage

• Not all clients will require or receive the maximum number of sessions

3. EXPIRATION POLICY

• All services must be used within the purchased timeframe (30 or 100 days)

• Unused sessions are forfeited after expiration

• Extensions are not guaranteed and are granted solely at Practitioner's discretion

4. PAYMENT & NO REFUND POLICY

• Payment is required in full prior to beginning services

• All sales are final

• No refunds will be issued under any circumstances

By purchasing, Client acknowledges commitment to the process and understands that results are not guaranteed.

5. UPGRADE POLICY

• Clients may upgrade from the 30-Day Intensive to the 100-Day Journey

• Any amount previously paid will be credited toward the upgraded service

• Upgrades must occur within the active 30-day period

6. CLIENT RESPONSIBILITY

Client understands and agrees that:

• Results depend on personal participation, openness, and follow-through

• Hypnosis is a collaborative process

• The Practitioner facilitates change, but the Client is responsible for their experience, integration, and outcomes

7. NO GUARANTEES

While hypnosis can be highly effective, results vary by individual.

The Practitioner makes no guarantees regarding specific outcomes.

8. NON-MEDICAL SERVICES DISCLAIMER

Soul Fresh Hypnosis services:

• Are not medical, psychological, or psychiatric treatment

• Do not diagnose, treat, or cure any condition

• Are not a substitute for licensed healthcare or mental health services

  
  

Client agrees to seek appropriate licensed care when needed.

9. CLIENT ELIGIBILITY & APPROPRIATENESS

Client acknowledges that hypnosis services are intended for individuals who are mentally and emotionally stable and capable of participating in a self-directed personal development process.

These services are not appropriate for individuals currently experiencing:

• Psychosis or psychotic disorders

• Severe mental health conditions requiring intensive or ongoing psychiatric care

• Active suicidal ideation or crisis states

Client confirms that:

• They are not currently experiencing the above conditions

• They are not relying on hypnosis services as a substitute for medical or mental health treatment

If Client is currently under the care of a licensed medical or mental health professional, Client agrees to consult with that provider if appropriate before engaging in services.

Practitioner reserves the right to decline or discontinue services if it is determined that the Client is not an appropriate fit.

10. RELEASE OF LIABILITY

Client voluntarily agrees to participate and assumes full responsibility for their experience.

Client releases Soul Fresh Hypnosis and Practitioner from any liability related to participation in services.

11. CONFIDENTIALITY

All client information and session content will be kept confidential, except where disclosure is required by law.

12. SCHEDULING & CANCELLATIONS

• Sessions must be scheduled in advance

• A minimum of 24 hours’ notice is required for rescheduling

• Missed sessions or late cancellations may be forfeited

13. PROFESSIONAL BOUNDARIES

Practitioner reserves the right to:

• Decline or discontinue services at any time

• Refer Client to other professionals if appropriate

14. AGREEMENT

Client acknowledges they have read, understood, and agreed to all terms outlined above. You acknowledge and agree that:

• You have read and understand this agreement in full

• You take full responsibility for your own healing and personal growth

• You understand that hypnosis is not a replacement for medical or psychological care

• You understand that results vary and no guarantees are made
